Please review the certification guidelines and criteria prior to applying for certification or re-certification as the fee is non-refundable. Please type your responses and return this document by email or mail with the required certification documents. This application will not be processed until the fee for certification or re-certification is received. Board Certification fee is $150.00. Re-certification is $25.00.

It is the applicant’s responsibility to provide all required certification or re-certification documentation with this application. Failure to complete this application fully, submit all required documentation and pay the required fee may, at the ASFP’s sole discretion, result in denial of certification and forfeiture of any fees paid. This application and any materials submitted with it will not be returned. Please keep a copy for your records.
